Same-Day Angiography, Cardiac Surgery Confirmed as Risk for Kidney Injury

Chicago Performing heart surgery on the same day as angiography is now confirmed as a risk factor for acute kidney injury (AKI), and hospital policy limiting the practice for elective cardiac surgery has significantly reduced the rate of AKI, according to a study published in the February 2013 issue of The Annals of Thoracic Surgery.

Although previous research has shown that performing diagnostic angiography and cardiac surgery on the same day is associated with increased risk of contrast-induced kidney injury, this study is the first to confirm causation and explore results following a change in hospital policy reducing this practice.

Researchers from Istituto di Ricovero e Cura a Carattere Scientifico (IRCCS) Policlinico San Donato in Milan, Italy, examined data from 4,440 patients who underwent cardiac surgery and received a diagnostic angiography prior to surgery.

Both cardiac surgery and the use of contrast media are risk factors for AKI. It appears that the combination of these factors on the same day significantly increases the risk, said lead author Marco Ranucci, MD.

AKI after cardiac surgery is a severe complication that is associated with a high risk of mortality. AKI comprises three stages of increasing severity from stage 1 to stage 3.

The researchers observed a significantly higher rate of AKI stage 1 in patients operated on the same day as angiography versus patients operated on 3 days later (18.5% vs. 13.5%, respectively). Additionally, the incidence of AKI stage 2 or 3 was significantly higher (7.8%) in patients who were operated on the same day as angiography than in patients operated on 1 day or 3 days after angiography (4.4% and 3.9%, respectively).

Overall, surgery on the same day as angiography was significantly associated with AKI stage 2 or 3 and any AKI, and an institutional policy limiting the practice of surgery on the same day as angiography resulted in a significant decrease in AKI stage 1 (30%) and a significant decrease of any AKI (42%).

We recommend that angiography on the same day as surgery should be avoided or at least limited to low-risk cases. In patients at high risk for AKI, such as elderly patients or patients with pre-existing renal dysfunction, surgery should be planned at least 24 hours after angiography, Dr. Ranucci concludes. Since no randomized controlled trials are possible in this area, recently released guidelines offer the best possible level of evidence.

Hospital policy can reduce risk of kidney injury
In an invited commentary in the same issue, Jeremiah R. Brown, PhD, MS, an Assistant Professor at The Dartmouth Institute for Health Policy and Clinical Practice at the Geisel School of Medicine in Hanover, NH, wrote that the results from Ranucci and colleagues presents a convincing case for setting hospital policy to limit the use of same-day cardiac catheterization for non-emergent cardiac surgery.

Dr. Brown said that Dr. Ranucci and his colleagues demonstrated great success in improving important patient outcomes and quality of care with a simple institutional policy change to eliminate the practice of same-day cardiac catheterization for non-emergent cardiac surgery.

In the meantime, Dr. Brown recommended that patients work with their physicians to reduce risk of kidney injury by planning to complete a cardiac catheterization ahead of time: Talk with your surgeon about having a diagnostic angiography at least 2 days ahead of planned cardiac surgery. There are rare circumstances for invasive diagnostic tests to be performed on the same day for non-emergent cardiac surgery. Hospital policies can be implemented to reduce the practice of same-day catheterization for cardiac surgery and minimize kidney injury, said Dr. Brown

Foundation Financial Group Plans Ribbon Cutting Ceremony for Launch of New Downtown Space


Foundation Financial Group will be hosting a Ribbon Cutting Ceremony for the acquisition of the 14th floor in the SunTrust Bank building. Official possession of the space is set to begin February 1st.

Sabrina Hall, Property Manager of Riverwatch Management, the leasing company for SunTrust, is optimistic about FFG moving in to their new office space. Having another growing consumer brand domiciled in downtown Jacksonville is critical for building momentum, she said. With a core private sector brand such as FFG, there will be advertisers, lawyers, IT shops and accountants that will want to be in close proximity.

It is very important for marquis private sector businesses to create a presence downtown, because that is ultimately the image of Jacksonville that will be projected to other geographies, Hall continued. The Gate Parkway and Southside areas are easy to duplicate in all Suburban parts of the United States. Downtown Jacksonville has the potential to be a magnet for Jacksonville as a whole, as it is highly differentiated in terms of its rich history, river views, and proximity to transportation infrastructure. Ultimately, this differentiated downtown will be the magnet in attracting talent, other companies, and investors into northeastern Florida.

With this added space, Foundation Financial Group is obtaining about 38,000 square feet of space. This will allow for about 150 people. In terms of jobs, this will allot room for 55 more jobs in Jacksonville. FFG will be increasing their square footage by 77 percent. Aside from this space picked up, there is more space available for even more growth.

Virginia Tech Expands Sports Concussion-Risk Studies to Include Hockey and Baseball

The Virginia Tech Wake Forest University School of Biomedical Engineering and Sciences is expanding its ground-breaking research of testing football helmets to reduce the number of concussions to now include hockey, baseball, softball, and lacrosse.

The five-year plan will see the Virginia Tech research center, headed by Stefan Duma, rate helmets worn by hockey, baseball, softball, and lacrosse athletes in their ability to lessen the likelihood of a concussion resulting from a violent head impact.

Ratings on hockey helmets are expected in fall 2013, followed by youth football in 2015, and then baseball, softball, and lacrosse in 2016. During that time, all ratings for adult and youth football helmets will continually be updated and released to the public.

The expansion into helmeted sports other than football comes on the heels of new research that allows for better prediction of sports-related concussions resulting from linear and rotational head accelerations. These accelerations result from head impacts that cause the head to translate and twist about the neck. The new research is published this month in the Annals of Biomedical Engineering.

New ratings for football helmets will include data for linear and rotational accelerations starting in 2015, said Duma, professor of biomedical engineering and department head of the Virginia Tech Wake Forest University School of Biomedical Engineering and Sciences. Serving as lead author on the research paper is Steven Rowson, assistant professor of biomedical engineering at Virginia Tech.

All head impacts result in both linear and rotational accelerations, and this publication provides the foundation for our research to address both accelerations relative to reducing the risk of concussion, said Duma. Our goal with the five-year plan is to provide manufacturers with a schedule detailing when we will release helmet ratings for each sport.

The helmet rating system is based on more than a decade of data collection by Duma and his research staff, and utilizes the STAR, or Summation of Tests for the Analysis of Risk, formula that assesses the ability of football helmets to reduce concussion risk. Sport-specific testing methodologies will be added to the website that lists the rated helmets prior to the initial release of each sports helmets ratings.

Using data collected from more than 63,000 head impacts during a period of 10 years, Duma and Rowson related linear and rotational head acceleration to the probability of sustaining a concussion in the form of an injury risk function.

This new analysis utilizes data measured from 62 concussions sustained by high school, college, and professional football players, said Rowson. We use these data to determine the best method to predict concussions when we test helmets in our laboratory.

In their research paper, the researchers write, With as many as 3.8 million sports-related concussions occurring annually in the United States and research suggesting possible long term neurodegenerative processes resulting from repetitive concussions, reducing the incidence of concussion in sports has become a public health priority.

Indeed, long-term, repetitive injuries that can cripple or eventually kill years after play have prompted dozens of headlines in major media outlets and several national studies, and even President Obama to recently weigh in on the subject. Dozens of former NFL players are suing the league over injuries sustained during years of play, and headlines were made this summer when former football great Alex Karras died at age 77 from various ailments, several allegedly said to be caused by years of hard hits.

In studying football-related injuries during the past decade, Duma and his research team have used on-field real-time sensors installed in the helmets of hundreds of adult and youth football players to study injuries, as well as a mechanical lab-tested 5-star rating system to track and grade commercially sold helmets.

The former can help indicate head injuries that require immediate attention while on the field of play. The latter has provided the only independent biomechanical data for consumers to make helmet purchasing decisions, Duma said.

Dumas goal is not to end the sport of football, but make it safer while still keep the same expected adrenaline rush and action for players, and viewers.

It is important to note that no helmet can prevent all concussions. The most effective strategies to reduce concussions in sports involve modifying league rules and player technique to limit exposure to head impacts, Duma said.

Beyond this, head impacts are a given in sport. Our research focuses on identifying helmets that reduce concussion risk so that athletes can make informed decisions based on independent data when purchasing equipment, which in turn, incentivizes helmet manufacturers to design helmets that better reduce head acceleration.

Debunking the July Effect: Surgery Date Has Little Impact on Outcome

ROCHESTER, Minn. -- The July Effect -- the notion that the influx of new residents and fellows at teaching hospitals each July makes that the worse time of year to be a patient -- seems to be a myth, according to new Mayo Clinic research that examined nearly 1 million hospitalizations for patients undergoing spine surgery from 2001 to 2008. Among those going under the knife, researchers discovered that the month surgery occurred had an insignificant impact on patient outcomes.

In addition, no substantial July Effect was observed in higher-risk patients, those admitted for elective surgery or patients undergoing simple or complex spinal procedures. The research was published online Tuesday in Journal of Neurosurgery: Spine.

We hope that our findings will reassure patients that they are not at higher risk of medical complications if they undergo spinal surgery during July as compared to other times of the year, says study co-author Jennifer McDonald, Ph.D., a Mayo Clinic radiologist. While we only looked at spinal surgeries, we think its likely wed find similar outcomes among other surgeries and procedures.

Researchers analyzed 2001 to 2008 data in the Nationwide Inpatient Sample, a large public database of information on hospitalized U.S. patients.

The researchers found that the incidents of all outcomes studied were higher in teaching hospitals, those with residents and fellows, than in nonteaching hospitals. In the teaching hospitals, minimally higher rates of postoperative infection and patient discharge to a long-term facility were found during July when compared with other months, but they were not high enough to establish a July Effect.

In-hospital deaths and postoperative complications did not differ according to the month of admission, the study found.

Lesbians, gays and bisexuals (LGBs) who are out to others have lower stress hormone levels and fewer symptoms of anxiety, depression, and burnout, according to researchers at the Centre for Studies on Human Stress (CSHS) at Louis H. Lafontaine Hospital, affiliated with the University of Montreal. Cortisol is a stress hormone in our body. When chronically strained, cortisol contributes to the wear and tear' exerted on multiple biological systems. Taken together, this strain is called allostatic load. Our goals were to determine if the mental and physical health of lesbians, gay men and bisexuals differs from heterosexuals and, if so, whether being out of the closet makes a difference. We used measures of psychiatric symptoms, cortisol levels throughout the day, and a battery of over twenty biological markers to assess allostatic load, explained lead author Robert-Paul Juster. Contrary to our expectations, gay and bisexual men had lower depressive symptoms and allostatic load levels than heterosexual men. Lesbians, gay men, and bisexuals who were out to family and friends had lower levels of psychiatric symptoms and lower morning cortisol levels than those who were still in the closet.

Montrealers of diverse sexual orientations were invited to the laboratory of Dr. Sonia Lupien, Director of the CSHS. Lupiens team recruited eighty-seven men and women, all of whom were around twenty-five years of age. Over the course of several visits, the researchers collected psychological questionnaires, asked participants to provide saliva samples to measure cortisol over two days, and calculated allostatic load indices using results from blood, saliva, and urine samples. Chronic stress and misbalanced cortisol levels can exert a kind of domino effect on connected biological systems, Lupien said. By looking at biomarkers like insulin, sugar, cholesterol, blood pressure, adrenalin, and inflammation together, an allostatic load index can be constructed and then used to detect health problems before they occur.

Stigma-related stress might force LGBs to develop coping strategies that make them more effective at managing future stressors. Coming out of the closet is a major milestone in lives of LGBs that has not been studied extensively using interdisciplinary approaches that assess stress biomarkers said co-author Dr. Nathan Grant Smith. These exciting findings underline the role self-acceptance and disclosure has on the positive health and wellbeing of LGBs. In turn, this has important implications for ongoing political debates. Coming out might only be beneficial for health when there are tolerant social policies that facilitate the disclosure process said Juster. Societal intolerance during the disclosure process impairs ones self-acceptance that generates increased distress and contributes to mental and physical health problems.

As the participants of this study enjoy progressive Canadian rights, they may be inherently healthier and hardier, Juster said. Coming out is no longer a matter of popular debate but a matter of public health. Internationally, societies must endeavour to facilitate this self-acceptance by promoting tolerance, progressing policy, and dispelling stigma for all minorities.

Women with harmful mutations in the BRCA gene, which put them at higher risk of developing breast and ovarian cancer, tend to undergo menopause significantly sooner than other women, allowing them an even briefer reproductive window and possibly a higher risk of infertility, according to a study led by researchers at UC San Francisco.

Moreover, the study showed that carriers of the mutation who are heavy smokers enter menopause at an even earlier age than non-smoking women with the mutation.

While the authors note that further research is needed, given the size and demographics of the study, women with the abnormal gene mutation should consider earlier childbearing, and their doctors should encourage them to initiate fertility counseling along with other medical treatments, the scientists said.

The study will be published online in Cancer on January 29, 2013.

This is the first controlled study to explore the association between BRCA1 and BRCA 2 and the age at onset of menopause, the authors said.

Our findings show that mutation of these genes has been linked to early menopause, which may lead to a higher incidence of infertility, said senior author Mitchell Rosen, MD, director of the UCSF Fertility Preservation Center and associate professor in the UCSF Department of Obstetrics, Gynecology and Reproductive Sciences. This can add to the significant psychological implications of being a BRCA1/2 carrier, and will likely have an impact on reproductive decision-making, Rosen said.

Mutations in either of the genes BRCA 1 or BRCA 2 can produce a hereditary, lifetime risk of developing breast cancer and ovarian cancer. Some women decide to reduce their risk by undergoing prophylactic surgery to remove at-risk tissue, including their breasts and ovaries. The abnormal genes are the most identified inherited cause of breast cancer carriers are five times more likely to develop breast cancer than those without the mutations, according to the National Cancer Institute.

The new study was designed to determine whether women with the BRCA1 or BRCA2 mutation have an earlier onset of menopause compared with unaffected women.

The researchers looked at nearly 400 female carriers of mutations in the BRCA gene in northern California and compared their onset of menopause to that of 765 women in the same geographic area without the mutation. Most of the women in the study were white because almost all of the BRCA1/2 carriers within the UCSF cancer risk registry are white.

The scientists found that women with the harmful mutation experienced menopause at a significantly younger age 50 years -- compared to age 53 for the other midlife women.

Heavy smokers (more than 20 cigarettes a day) with the abnormal gene had an even earlier onset of menopause -- 46 years. By comparison, only seven percent of white women in northern California had undergone menopause by that age, said the authors. Smoking has been shown to alter menstrual cycles and estrogen status, among other impacts.

The authors point out that while their study shows a possible increased risk of infertility for the mutation carriers, further study is needed. They also said that data regarding the age of natural menopause is limited because most women with the mutation are recommended to undergo risk-reducing surgery after they complete childbearing.

Women with the mutation are faced with challenges in reproductive choices, said study co-author Lee-may Chen, MD, a professor in the UCSF Department of Obstetrics, Gynecology and Reproductive Services. These data may help women understand that their childbearing years may be even more limited by earlier menopause, so that they can make decisions about their reproductive choices and cancer risk-reducing surgery.

The first author of the study is Wayne T. Lin, MD, MPH, who at the time of the research was a resident at UCSF and is now a fellow at the Brigham and Womens Hospital at Harvard Medical School. Other authors include Marcelle Cedars, MD, a UCSF professor and director of the Division of Reproductive Endocrinology in the Department of Obstetrics, Gynecology and Reproductive Services ; and Mary Beattie, MD, clinical professor in the UCSF Department of Medicine. Study data was collected from the Cancer Risk Program at UCSF and the northern California site of the Study of Womens Health Across the Nation, a project of the University of California Davis and Kaiser Permanente.

Incurable Cancer: New Book "I Am Living to Tell" Explains How One Woman Beat It and Remains Cancer-Free Seven Years Later

Forty-two year old Laurie Beck enjoyed a busy life as the successful owner of a Pilates studio, a wife, and mother, but then things took a dramatic turn -- she learned that she had lymphoma and an incurable form of cancer. Today however, nearly seven years after her diagnosis, repeated tests show no signs of cancer in Becks body.

How did Beck beat cancer? In her inspirational new memoir, I Am Living to Tell, Beck describes her healing journey, which began with her decision not to pursue chemo right away and to practice watchful waiting instead, something her oncologist at Bostons Dana-Farber Cancer Institute recommended as long as she felt well. Beck recounts her efforts to heal herself by developing a healthier mind, body and spirit using a combination of Western and Eastern medicine and alternative healing modalities. For example, Beck:
  • Let go of the unhealthy feelings of resentment, anger and abandonment she had been harboring for decades toward her mother, grandmother and ex and learned to forgive.

  • Focused all of her intentions on getting well using meditation, prayer and, prayed, and positive visualization.
  • Surrounded herself with love.
  • Began to pay close attention to what she put in her body; developed a healthier approach toward exercise; and pursued healing forms of exercise, like Qigong.

Bottom-line, Beck learned what she had done in her pre-cancer life to create her bad health. Then, she healed herself by making peace with her past, doing everything she could to rid her body of chronic stress, learning to love herself, and believing in her own power.

Anyone struggling with a difficult health challenge will draw strength and inspiration from Becks healing journey and the lessons she learned about herself and life along the way. To help them pursue their own journeys toward better health, Beck ends her book with a chapter that discusses tips and tools that can help them heal their minds, bodies and spirits.
